Both AC connections to the 6SN7 heater rectifier were completely bare, as was one end of the coupling cap on the 2A3. I can say almost 100% that was the cause of the problem, since the leads were not only unsoldered, but were also quite loose. I'll get those interconnects made and give them a try.
